Hawai`i County Office of Aging <br /> Chronic Disease Self Management Program — ARRA funds <br /> Draft Scope of Service <br /> a. Evaluate trainee mastery and readiness to implement CDSMP, DSMP, and /or ASMP. <br /> b. Utilize the Aging and Disability Resource Center ( "ADRC ") to identify older adults with <br /> chronic diseases who will be offered CDSMP, DSMP, and /or ASMP. <br /> c. Offer six (6) cycles of CDSMP, ASMP, and /or DSMP. Engage a sufficient number of <br /> participants in CDSMP, ASMP, and /or DSMP so that, during the period of Contract <br /> performance, at least sixty (60) participants complete at least four (4) out of the six (6) <br /> sessions that make up a cycle. <br /> d. Identify sites for CDSMP, ASMP, and /or DSMP workshops in the targeted communities <br /> of Kohala and Hamakua. <br /> e. Recruit and train Lay Leaders to deliver workshops with fidelity in the targeted <br /> communities. <br /> f. Identify sites for CDSMP, ASMP, and /or DSMP workshops in the targeted communities <br /> of Kohala and Hamakua. <br /> g. Recruit and train Lay Leaders to deliver workshops with fidelity in the targeted <br /> communities. <br /> h. Monitor with fidelity the workshops delivered. <br /> i. Participate in evaluation activities with the STATE's contracted evaluator. <br /> j.
