Requesting a resolution to authorize the Mayor to enter into an agreement with the State Department of <br /> Health (DOH) to receive federal grant funds from the American Recovery and Reinvestment Act of 2009 <br /> (ARRA) for the Chronic Disease Self Management Program (CDSMP) in Hawaii County. <br /> 2. Requesting a bill for an ordinance: <br /> Appropriation: $25,496.00 CDSMP - ARRA <br /> Revenue: $25,496.00 <br /> B. BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED): <br /> The Hawaii County Office of Aging (HCOA) has been awarded ARRA grant funds from Dept. of Health, <br /> Executive Office on Aging, for expansion of the Chronic Disease Self Management Program (CDSMP.) <br /> Accountability and transparency are key components of the ARRA and therefore the funds must be tracked <br /> separately from other federal grants received for the CDSMP. The ARRA funds will enable HCOA to expand <br /> CDSMP workshops to Kohala and Hamakua (see attached Draft of scope of service submitted for proposal.) <br /> Request to be waived from Finance Committee - 1S year funds to be expended by March 30, 2011; HCOA to <br /> contract out funds. <br /> SIGNED: � ' DATE: 09/15/10 <br /> Department Head <br />
