Transportation <br /> Provide transportation service to congregate meals participants in order for them to <br /> receive nutritious meals and participate in site activities that enhances physical and <br /> social well-being. <br /> a. The percentage of participants identified as non-nutritional risk at the <br /> beginning of the program year shall be maintained at the same or higher levels <br /> by the end of the program year. <br /> b. The percentage of participants identified as high nutritional risk at the <br /> beginning of the program year shall be maintained at the same or lower levels <br /> by the end of the program year. <br /> During the first six months of Fiscal Year 2011-12,229 unduplicated older <br /> individuals were provided with transportation services to participate at a <br /> congregate meal site. On the average, 168 older individuals received <br /> transportation service every month. Approximately 90% of participants <br /> completed a nutritional risk survey per month. In the same reporting period, <br /> the average monthly proportion of participants who were identified as <br /> nutritionally at-risk was 26%while 64% was the average monthly proportion of <br /> participants who were identified as non-nutritionally at-risk. <br /> The proportion of participants who were nutritionally at-risk in July 2011 (25%) <br /> increased by 3 percent at the end December 2011 (28%). The proportion of <br /> non-nutritionally at-risk stayed the same from July 1,2011 through December <br /> 31,2011. This year's service data clearly show that the nutritional status of <br /> recipients of transportation services were maintained within the acceptable plus <br /> or minus 10 variance points,which are the limits set by the State of Hawai`i's <br /> Executive Office on Aging. <br /> Contracts to the Elderly Activities Division,Department of Parks & Recreation. <br /> Funds provided to the Elderly Activities Division are deposited directly to its respective <br /> accounts.
