Transportation to enable older adults to access community services by providing <br /> transportation to/from congregate meal sites, medical and health related visits, <br /> grocery shopping and other community resources. <br /> a. A minimum of 350 older adults received transportation services to congregate <br /> meal sites and other community resources. 80% are satisfied with the serviced <br /> received. <br /> 4. Public Education will offer meaningful information about the services and benefits <br /> available to older adults and their caregivers living in Hawai'i County. <br /> a. A minimum of 4,200 householders,physician offices, and organizations receive <br /> the monthly publication called the Silver Bulletin. As a result, at least 75%report <br /> satisfaction with the information content, quality and relevance of the newsletter <br /> as well as usefulness of the newsletter regarding benefits and programs around <br /> Hawai`i County. <br /> Develop Hawai'i's Aging and Disability Resource Center(ADRC) to its fully <br /> functioning capacity to serve as a highly visible and trusted place where all persons <br /> regardless of age, income and disability can find information on the full range of long- <br /> term care support options. <br /> 1. Aging and Disability Resource Center(ADRC) Development Project <br /> a. The ADRC will provide information and assistance services to a minimum of <br /> 1150 of older adults and their caregivers (unduplicated). As a result at least 70% <br /> will be connected to appropriate services and resources. <br /> b. On an annual basis, the surveys will be sent to participants to determine their level <br /> of satisfaction with services provided. 75% of seniors and their care givers will <br /> report satisfaction with services. <br /> Ensure the rights of older adults and prevent their abuse, neglect and exploitation <br /> 1. Elder Abuse and Neglect Awareness to a minimum of 200 elders, their caregivers <br /> and professionals in the Aging Network increase their knowledge of and ability to act <br /> on elder abuse and neglect issues. <br /> 2. Legal Services a minimum of 300 individuals receive information about legal advice, <br /> counseling, and representation. As a result, 80% are linked to appropriate legal <br /> resources. <br /> 5 <br />
