Develop a Memorandum of Agreement with at least one acute hospital facility to <br /> streamline ADRC referrals and service access for eligible individuals. <br /> A Memorandum of Agreement is still pending Hilo Medical Center's legal <br /> review and acceptance. Transition service to Hilo Medical Center patients <br /> have continued with at least weekly ADRC staff visits with discharge <br /> planners. Though a Memorandum of Agreement with 'Ohana Health Plan, <br /> the ADRC receives direct referrals for transition services for their members <br /> discharged from Hilo Medical Center. <br /> e. Develop a working plan for updating resource database and expand use of Social <br /> Assistance and Management System program software capacity to include Aging <br /> and Disability Resource Center service delivery documentation. <br /> State grant funding to update the resource database was approved for a <br /> limited term staff position. The Social Assistance and Management System <br /> (SAMS) program is used for Aging and Disability Resource Center <br /> documentation. ADRC staff are receiving training to use web based SAMS <br /> intake,in-home assessment, and support plan tools. <br /> 2. Caregiver Support: National Family Caregiver Support Program <br /> a. Develop and maintain information and outreach displays and presentations about <br /> the availability of support services in the community through public education in <br /> group settings (health fairs, community service fairs, or community organization <br /> meetings). <br /> • A minimum of one presentation will be given annually in each of <br /> nine districts island-wide. <br /> • HCOA will participate in at least two health and/or resource fairs, <br /> one in East Hawai`i and one in West Hawai`i. <br /> During the first six months of FY13,the Hawaii County Office of Aging <br /> coordinated 5 caregiver conferences and workshops for over 130 caregivers <br /> and members of the public. They included "Caregiving 101" held at Parker <br /> Ranch School in Waimea, spearheaded by North Hawaii Hospice; "Poetry <br /> for Caregivers" held at the Regency at Hualalai featuring Frances <br /> Kakugawa; and monthly "Caregiver Respite Fun Days" held at Hawaii <br /> Island Adult Care" featuring Frances Kakugawa, Audrey Wilson, and Lori <br /> Thal, among others. <br /> Six Month Progress Report on Program Objectives for FY 2012-13 2 <br />
