AGING <br /> AGING <br /> Conferences include keynote speakers, breakout presentations, and <br /> information on services, programs, and resources available for caregivers in <br /> Hawaii County. Presentation topics included: knowing the basics of <br /> caregiving, a caregivers' story, home health care services, caring for loved <br /> ones with dementia,legal and psychological issues of caregiving, easing the <br /> burden of Alzheimer's Disease through journaling and poetry, caregiver <br /> support,tai chi and other relaxation techniques, healthy cooking <br /> demonstrations, art therapy, and hands-on caregiver skills training. <br /> b. Provide access to all districts of the island, trained individuals to assess caregiver <br /> needs,provide information and assistance that link caregivers to needed services <br /> and provide follow-up on an as-needed basis. <br /> At least 80%of requests for information and/or services will have immediate <br /> (within 7 days) contact with an assessor to begin the assessment process and <br /> identify caregiver needs. <br /> The Hawaii County Office of Aging contracted with Services for Seniors, <br /> Inc. to provide access and caregiver assessment services for family <br /> caregivers. Caregiver assessments and care coordination services assist <br /> caregivers in obtaining access to Home and Community Based Services. <br /> FY13 year to date services provided include: 371.5 units of access services to <br /> 46 family caregivers and 12 caregiver assessments for 12 family caregivers. <br /> An annual survey to collect caregiver satisfaction data will be completed by <br /> September 30,2013. <br /> c. Provide respite, support group, and counseling services for caregivers based on <br /> needs identified through the Montgomery-Borgatta Burden Scale screening tool. <br /> Respite services will include adult day care,personal care, and homemaker <br /> services. <br /> At least 80%of caregivers with needs identified will receive respite, support <br /> group, or counseling services. <br /> The Hawaii County Office of Aging contracted with 14 service providers to <br /> provide Home and Community Based Services for family caregivers which <br /> include: respite care, counseling, and supplemental services. FY13 year-to- <br /> date services provided include: 8 family counseling sessions, 1,292 units of <br /> respite care for 19 family caregivers, and 68 units of supplemental services to <br /> 39 caregivers.
