AGING <br /> AGING <br /> 3. Community Planning and Development—Partnerships <br /> Healthy Aging Initiative <br /> a. Increase by 2 the number of public and private agencies that will be members of <br /> the Hawai`i County Healthy Aging Partnership coalition. The increase would be <br /> based on the number of partners at the end of fiscal year ending June 2012. The <br /> intent is to embed a health related evidence-based program in Hawai`i County by <br /> building capacity and supporting healthy aging activities. <br /> Although the Big Island Healthy Aging Partnership did not increase its <br /> agency membership for the first 6 months of FY 2013,the ARC of Hilo and <br /> the Unitarian Church of Puna increased capacity with the addition of 5 new <br /> leaders. <br /> b. Support and coordinate the Better Choices, Better Health—Ke Ola Pono Program, <br /> a chronic disease self management evidence-based community program. Deliver <br /> a minimum of 10 workshops serving 100 participants. <br /> Due to a certification process, the 5 new Lay Leaders trained in June 2012 <br /> need to complete a total of six workshops. One workshop was offered at <br /> Tutu's House in Waimea in October 2012 and the second workshop took <br /> place at the Aging and Disability Resource Center in Hilo in November 2012. <br /> A total of 17 participants completed the workshops. Three more workshops <br /> are scheduled to begin in January 2013. <br /> 4. Education/Training <br /> Support and coordinate a minimum of two (2) workshops and/or educational seminars <br /> for Aging and Disability Resource Center (ADRC) clients and/or service providers <br /> that reflect identified needs of clients and providers. <br /> Presentation was made at the September 2012 Hawaii Pacific Gerontological <br /> Society Conference on ADRC's Care Transition Intervention (CTI) services. <br /> Access to webinars on Medicare Updates provided through Executive Office on <br /> Aging SAGE Plus program was coordinated by the Office of Aging. <br /> Workshops, classes and presentations completed July 1,2012-December 31,2012 <br /> ➢ July 2012 SafeTALK Suicide Prevention Training—ADRC <br /> ➢ September 2012 SafeTALK Suicide Prevention Training—WHCC <br /> ➢ September 2012 SafeTALK Suicide Prevention Training—ADRC <br /> ➢ October 2012 Medicare Update Training—ADRC <br /> Six Month Progress Report on Program Objectives for FY 2012-13 4 <br />
