Fully-Operational Aging and Disability Resource Center <br /> a. By June 2018, HCOA's ADRC will be fully-operational, all while the State <br /> honors HCOA's business and clinical model of services, and ADRC staffing will <br /> parallel the demand for services. <br /> b. By June 2018, a 2%increase in call-ins and walk-ins will be expected at both the <br /> East and West Hawai`i ADRC. <br /> Developing a statewide ADRC system for older adults and their families to access and <br /> receive Short/Long Term Support Services (S/LTSS) within their respective counties. <br /> 3. Improved access to Aging and Disability Resource Center Services <br /> a. By June 2018, 8-out-of 10 individuals (80%) who make contact with any of the <br /> ADRC sites will have their questions and concerns answered to their liking, and <br /> 9-out-of 10 individuals (90%).will perceive services as delivered in a professional <br /> manner with respect and aloha. <br /> Enablingpeople with disabilities and older adults to live in their communitythrough <br /> P P <br /> the availability of and access to high-quality Short/Long Term Services and Supports, <br /> including supports for families and caregivers. <br /> 4. Home and Community Based Services and Caregiver Support <br /> a. By June 2018, HCOA (through its case management contracted provider) will <br /> meet the service needs of 350 consumers of which at least 80%will remain in <br /> their home three months following formal services. <br /> b. By June 2018, caregiver supports and services will increase by 2%. In 2018, <br /> HCOA will reach the following caregivers: 300 conference participants; at least <br /> 50 hours of counseling,peer support groups and training; 50 caregivers receiving <br /> respite services; and at least 25 consumers receiving care giving supplemental <br /> services. <br /> 4 <br />
