5. EMPLOYEE PARKING <br /> 5.1.Application procedures <br /> 5.1.1. An eligible employee seeking a parking permit shall obtain an <br /> Application Form from the parking control officer or the <br /> Administration office of the Department of Public Works. Each <br /> applicant shall fully and accurately complete the form and shall <br /> provide all information requested by the county, including, but not <br /> limited to: <br /> 1. Name, telephone number, work place address, and <br /> government agency; <br /> 2. Registered owner(s) of motor vehicle(s); <br /> 3. Year, make and license number of primary motor vehicle, if <br /> applicable; <br /> 4. Year, make and license number of secondary motor vehicle, if <br /> applicable; <br /> 5. Facility working in; and <br /> 6. Type of parking requested. <br /> 5.1.2. Employees who are an individual with a disability who is <br /> qualified and wish to request accessible parking spaces shall <br /> submit their requests to their departments/agencies as outlined in <br /> Procedures for Providing Reasonable Accommodation, <br /> Requesting Accommodations, 3. Employees b. with Application <br /> Form. <br /> • The employee and his/her supervisor, manager or other <br /> employee responsible for deciding whether a request for <br /> accommodation is reasonable shall engage in the interactive <br /> process, as necessary, to determine the what type of parking <br /> space will best meet the employee's need. <br /> • The departmental Accommodation Resource Manager shall <br /> inform the Administration Office, Public Works Department of <br /> the employee's need for accessible parking and submit the <br /> completed Application Form. <br /> 8of16 <br />
