• <br />ADDENDA, BULLETIPJS •AND INTERPRETATIOi1S - No interpretation of the meaning <br />of the plans, specifications or other pre-bid documents will be made to any <br />bidder orally before the opening of bids. Discrepancies, omissions, or doubts <br />as to the meaning of drawings and specifications should be communicated in <br />writing to the Engineer for interpretation. <br />Every request for such interpretation should be in writing addressed to <br />CHIEF ENGINEER. DEPARTT NT OF PUBLIC WORKS _, at 2 AUPUNI STREET, HILO. HAWAII9672q. Any interpretation, if made, and any supplemental instructions will be <br />in the form of written addenda to the specifications which, if issued, will be <br />mailed to all prospective bidders (at the respective addressesuf'uiteded..fbr <br />such purposes), not later than five (5) days prior to the date fixed for the <br />opening of bids. Failure of any bidder to receive any such addendum or inter- <br />pretation shall not relieve such bidder from any obligation under his bid as <br />submitted. All addenda and bulletins so issued shall become part of the <br />contract documents. - <br />2.6 C0r?TENTS OF PROPOSAL FORMS - Prospective bidders will be furnished with <br />proposal forms giving the location, description, and the contract time of the <br />work contemplated for which a lump sum bid price is asked or containing a <br />schedule of items, together with estimated quantities of work to be performed <br />and materials to be furnished, for which unit bid prices and/or lump sum bid <br />prices are asked. <br />?,,.;___ PREPARATION 0F. „PROPOSAL - The bidder's proposal must be submitted on the <br />proposal form furnished by the Department. The proposal must be filled in, <br />in full accordance with the instructions thereon. The bidder must state the <br />prices (written in ink or typed, both in words and numerals) for which it is <br />proposed to do each item of work contemplated. The bidder shall sign the <br />proposal in the spaces provided. If the proposal is made by an individual, <br />his name and post office address must be shown, If made by a firm or partner- <br />ship, the name and post office address of each member of the firm or partner- <br />ship must be shown, If made by a corporation, the proposal must show the name, <br />titles and business address of the president, secretary, and treasurer and <br />also evidence showing the authority of the bidder to enter on behalf of said <br />corporation into contract with the County in the form of a corporate resolution. <br />If made by a joint -venture, the name and post office address of each member of <br />the individual firm, partnership, or corporation comprising the joint -venture <br />must be shown with other pertinent information required of firms, partnerships <br />or corporations as the case may be Bids signed in behalf of a corporation <br />must be impressed with corporate seal if the corporation has a seal. If the <br />corporation has no seal,this fact should be noted. <br />PPOPOSAL GUARANTY - No proposal will be considered unless accompanied <br />by one of the following forms of bidder's security; a cashiers check or <br />certified check on a solvent bank doing business mithin the State or a bidder's <br />bond executed by an approved surety insurer; made payable at sight to the Eayor <br />in the manner, form and amount required by Section 103-28 to 103-31 inclusive; <br />Hawaii Revised Statutes. The proposal guaranty shall be equal to five percent <br />(55) of the total amount of the bid submitted provided, however, that if the <br />amount of the bid exceeds FIFTY THOUSAND DOLLARS (50;000.00), the said surety <br />bond; cashiers check or certified check shall be for 111:1O THOUSAND FIVE <br />HUNDRED DOLLARS ($2:500.00) plus two percent (2%) of the amount in excess of <br />Section 2 <br />Pate 8 <br />
