for a period of three (3) years after termination <br />of assistance or being declared ineligible. <br />g. Any member of the Family has violated any HUD <br />regulation or has failed to cooperate in the <br />administration of the program. If the violation <br />involves family obligations (as listed on the <br />voucher) other than those listed above, the family <br />will be ineligible for three (3) years from the <br />date that assistance is terminated. <br />h. Any member of the Family is subject to a lifetime <br />registration requirement under a State sex <br />offender registration program. A family will not <br />be denied assistance or terminated from assistance <br />provided that the family member subject to the <br />requirements is not a member of the household. <br />i. Any member of the Family fails to fulfill its <br />obligations under the Section 8 Welfare-to-Work <br />voucher program. In this case, the family will be <br />ineligible for three (3) years. <br />j. An Applicant who is or was part of a currently <br />Participating Family in the Agency’s homeownership <br />program and is currently included as part of that <br />household for occupancy and subsidy purposes. <br />H. APPLICATION FOR PROGRAM <br />1. Application: Each family shall be required to complete <br />and sign an OHCD application form for admission to the <br />program. Each application shall reflect the date and <br />time received and is numbered accordingly. <br />2. Review and Placement: The OHCD shall review the <br />application and place the family onto the waiting list <br />if there are no TBRA vouchers available. <br />3. Record of Family: A file folder shall be established <br />for each family and shall contain the application and <br />all data collected to verify and document the family's <br />status. <br />4. Misrepresentation: If any applicant deliberately <br />misrepresents the information on an application, the <br />OHCD may refuse to consider his/her application and may <br />turn over such matters to the proper authorities for <br />Rules and Regulations <br />-34- <br />0010ARslh-TBRA <br />11/01/2007 <br /> <br />
