Determination of Ineligibility: If an Applicant is <br />determined to be ineligible, the OHCD shall promptly <br />state the reasons for its finding in a letter to the <br />Applicant. The letter shall also state that the <br />Applicant may request an informal review of the <br />decision. <br />I. ESTABLISHMENT OF A WAITING LIST <br />1. Placement onto Waiting List <br />a. Each family shall be placed onto the waiting list <br />in accordance with the date and time of the <br />receipt of their application at OHCD or any office <br />authorized by OHCD to receive applications. <br />b. Each family file shall be maintained in a manner <br />so as to assure compliance with the selection <br />provisions set forth in Paragraph K. <br />c. Duplicate applications will not be accepted by the <br />OHCD. <br />d. Ineligible families will not be placed on the <br />waiting list. <br />2. Changes in Family Composition While on Waiting List: <br />If there is a change in family composition, the family <br />shall report the change, in writing, to the OHCD. An <br />applicant may not assign their application’s date and <br />time to someone who was not on the applicant’s original <br />application. <br />3. Changes in Family Address and Contact Information: If <br />there is a change in the family address or contact <br />information, the family shall report the change, in <br />writing, to OHCD. <br />4. Split Households Prior to Voucher Issuance: When a <br />family on the waiting list splits into two otherwise <br />eligible families due to divorce or legal separation, <br />and the new families both claim the same placement on <br />the waiting list, and there is no court determination <br />on this issue, the OHCD will decide which party will <br />remain on the waiting list, taking into consideration <br />the following factors: <br />Rules and Regulations <br />-35- <br />0010ARslh-TBRA <br />11/01/2007 <br /> <br />
