a.Which family member applied as head of household; <br />b.Which family retains the children or any disabled <br />or elderly members; <br />c.Restrictions that were in place at the time the <br />family applied; <br />d.Role of domestic violence in the split; <br />e.Recommendations from social service agencies or <br />qualified professionals such as children’s <br />protective services; and <br />f.Written notification from the applicant to <br />voluntarily replace their name on the application <br />with the name of another adult member who was on <br />the original pre-application. <br />Documentation of these factors is the responsibility of <br />the applicant families. If either or both of the <br />families do not provide the documentation, they may be <br />denied placement on the waiting list for failure to <br />supply information requested by the PHA. <br />5. Closing the Waiting List: OHCD may close the waiting <br />list to any further applications if the number of <br />applicants already on the waiting list is larger than <br />the number of units that could reasonably be <br />accommodated in one (1) year. The decision to close <br />the waiting list will be publicly announced. <br />6. Ineligibility for Waiting List: Applicants who apply <br />to be on the waiting list may be declared ineligible <br />for the waiting list if they refuse an offer of a <br />voucher, derive benefits fraudulently, have a history <br />of alcohol abuse, drug related criminal activity or <br />violent criminal activity (including criminal activity <br />for manufacture, production, or sale of <br />methamphetamines), have threatened abusive behavior <br />toward PHA personnel, have a history of repeated minor <br />violations or singular serious violation of the lease <br />provisions, a household member is subject to a lifetime <br />registration requirement under the State or Federal sex <br />offender registration program (a family will not be <br />denied assistance or terminated from assistance <br />provided that the family member subject to the <br />requirements is not a member of the household); have <br />violated any HUD regulation or have failed to cooperate <br />Rules and Regulations <br />-36- <br />0010ARslh-TBRA <br />11/01/2007 <br /> <br />
