The County of Hawai'i was awarded grant funds <br /> from the Administration on Aging to coordinate the mobilization of the public and private partnerships <br /> at the community level to implement and sustain the delivery of evidence-based prevention through <br /> an aging service provider organization. The Hawai'i County Office of Aging (HCOA) contracted with <br /> the Hawai'i County Nutrition Program (HCNP) to implement EnhanceFitness, an evidence-based <br /> exercise program, in two nutrition sites. For a number of reasons EnhanceFitness did not prove to <br /> be a very good fit for the nutrition sites, and a difficult decision was made to terminate the program. <br /> The HCOA is committed to maintaining or improving the health status of Hawai'i County's elderly <br /> citizens. To this end, HCOA requested approval from the State Executive Office on Aging (EOA) to <br /> explore the possibility of implementing the Stanford University Chronic Disease Self Management <br /> Program, another grant approved evidence-based prevention.
