Isa PHONE: 961 -8600 <br />A. REQUEST: <br />To enter into a multi -year lease agreement for five (5) years, for a multi- function copy machine in our Hilo <br />office. <br />B. BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED): <br />The Office of Aging is looking for a comparable copier with a higher capacity, to meet the steady <br />increase /demand in the office, as a replacement. <br />The current copier lease has expired on 09/30/2010. We now own the copier, however, errors message occur <br />and copy quality continues to decline. We have not purchased any monthly maintenance on our current <br />copier. <br />We have sufficient funds budgeted for the fiscal year 2011. <br />Our lease on the copier that has expired was $170.52 per month with an additional per copy charge of $.0082. <br />The average monthly cost for this copier was about $217 per month. <br />We would lease a copier for a period of five (5) years at a cost not to exceed $200 per month, and in addition to <br />the monthly lease, the cost per page of black and white copy shall not exceed the charge of $.008,5 per copy, <br />and the cost per page of color copy shall not exceed the charge of $.065 per copy. <br />SIGNED: DATE: ll <br />Department Head <br />
