Name: Kona Adult Day Center, Inc. <br /> Geographic Area Served: North, Central and South Kona. <br /> Kona Adult I)ay Center Mission: <br /> "Our mission i.c to provide health,social and recreational programs and activities that enable <br /> impaired adults to experience feelings of belonging,friendship,acceptance,accomplishment, <br /> and independence in a caring and secure social environment." <br /> 2. Programs: Kona Adult Day Center(KADC) programs encompass several types of services, <br /> ranging from social and recreational, to medical care, therapy and helping the individual maintain <br /> the highest level of functioning. The National Council on Aging defines adult day care as: <br /> "A community-based group program designed to meet the needs of functionally impaired adults <br /> through an individual plan of care. It is a structured, comprehensive program that provides a <br /> variety of services in a protective setting during any part of a day less than 24 hour care. <br /> Individuals who participate in day care attend on a planned basis during specified hours'. Adult <br /> Day Care assists its participants to remain in the community,enabling families and other caregivers <br /> to continue caring for an impaired member at home." <br /> Advocates of adult day care use the terms "continuum of care" and long-term care in describing the <br /> need for adult day care services in our communities. Adult day services provide care at a <br /> maintenance level,that is,limited but ongoing assistance and supervision at a fraction of the cost of <br /> either private hire nurses and private nursing home facilities. The cost effectiveness of adult day <br /> centers makes them a very important component in the continuum of care in West Hawaii. Kona <br /> Adult Day Center is the only adult day care facility on the west side of the Big island. <br /> 3 . Support: KADC services support the agency mission by providing each participant with an <br /> individualized plan of care. Activities programming include: arts and crafts, music therapy, group <br /> reminiscent therapy,pet and garden therapy,special projects,excursions, intergenerational therapy <br /> and group exercise. Health programming includes: individualized exercise and range of motion, <br /> care coordination, medication administration, health monitoring and supervision, nursing <br /> procedures, bowel and bladder training, hot lunches and snacks, massage and shiatsu, individual <br /> care planning and discharge planning, referral and information to families on other community <br /> services and providers of care. The health program is supervised by a registered nurse, certified in <br /> rehabilitation nursing. The activities program is supervised by an accredited activity director. <br /> Kona Adult Day Center offers a monthly caregiver support group, organized to help caregivers <br /> share ideas,frustrations and achievements. <br /> 4. Eligibility: Persons eligible for KADC services are those adults, 18 years and above, <br /> particularly those 60 and older,having either a physical or cognitive disability,requiring protective <br /> or supportive care. Participants must be able to function in a group setting without disruption. <br />
